Note:

Original documents must be brought and photocopied once.
Parents must be accompanied by their parents when applying for a child's passport.
Divorce certificate and child custody determination if the parents are divorced.
If the child was born abroad, the Indonesian Citizen Birth Report issued by the Civil Registration Office (Disdukcapil) must be attached.
The officer has the right to request additional documents if necessary.
